Bottom line first: most "failed" applications are not refusals — they're returned so you can fix or add documents. Seven common causes; avoid them and your chances of first-time approval go way up.

When a first-time applicant is turned away at the window, the immediate thought is "I've been refused." In fact, returned ≠ refused: a return means something in your documents needs fixing or supplementing; a refusal is a formal denial by the visa officer and is rare. Steer clear of the seven pitfalls below and your odds look completely different.
48mm×33mm, white background, color, bare-headed, taken within the last 6 months — simple rules, but this is the single biggest cause of returns. Common problems: over-edited AI beautification, bluish or grayish background, glasses or earrings, digital upload not matching the printed copy. We covered the details earlier — check your photo against that list before you go.
Family-visit letters missing the inviter's ID number or who covers the expenses; business letters missing the company seal; travel dates or length of stay in the letter not matching the COVA form — the counter staff check all of these. See our invitation-letter guide and go through the checklist item by item.
This is the most frustrating kind of return: a typo in the date of birth, passport number, or travel dates on the form, or an itinerary that doesn't match the flight booking. Before submitting, go through the form line by line — ten minutes with a printed copy can save a wasted trip.
Your passport generally needs to be valid for at least 6 more months with enough blank visa pages. Many people prepare everything else perfectly and get caught on this one — check your expiry date before you apply, and renew the passport first if it's short. See our passport renewal guide.
Applying for an L (tourist) visa with only a flight ticket and no hotel bookings, or an M (business) visa with no credible visit plan. The logic is simple: your documents must prove that your stated purpose of visit is real. If your itinerary isn't fixed, the invitation-letter route is more flexible — see the invitation-letter guide.
Employed applicants need an employment letter or certificate; students need a student ID or enrollment letter. The window wants to see you have a stable status and ties in the US — an important factor in assessing whether you'll depart on time.
Past overstays or refusal stamps in an old passport don't mean automatic refusal, but you may well be asked about them. Prepare your explanation and supporting documents in advance and answer truthfully — far better than being caught off guard.
